Replacement or Lost Badge Form:

• • If you lost your badge or have a defective badge,  click below to complete form and payment. 

  • Lost badge has a replacement cost of $25.00. 

  • If you have a defective badge, you must return your badge before a new one is issued.

  • Send a picture (headshot) taken against a light background to ecclpics@gmail.com.
